«Three wrong codes: the request was declined.»You typed a code different from the browser's three timesAsk again and copy the code carefully

In the browser, «The request is no longer waiting for approval.» means the request was already answered, declined or expired.

Why must the code match?

The code shown in the browser proves the request on the phone is the same one you just made. If you see no code in the browser, you did not make this request: decline it with "It wasn't me".

When to escalate

A request that cannot be verified, or requests you did not make, must be reported at once to your organization's security team.
